Application of autologous costal cartilage in rhinoplasty revision

Abstract：Objective To summarize the application experience and complications involved in the use of autologous costal cartilage in rhinoplasty revision.Methods From 2015 to 2016,the data of 36 patients who received autologous costal cartilage for rhinoplasty revision ware analyzed retrospectively.Along the right mammary inferior fold incision,partial anterior sheath of rectus abdominis,rib perichondrium,and autologous costal cartilage (6th or 7th) ribs were cut.The nasal cartilage structure was exposed through an open approach.The capsule or hyperplastic scar was removed,and the skin and soft tissue of the dorsum nasi cover was thoroughly released.The nose was repaired comprehensively.The nasal tip was reconstructed with autologous material,and the dorsum nasi shape was remodeled.Retrospective statistical analysis was made of complications such as deformation,infection,graft absorption,and allograft rejection.The subjective scores of the cosmetic results and functional satisfaction (quadruple scale) were recorded.Results The patients were followed up from 6 to 24 months.In all cases,the shape of the dorsum nasi was natural and smooth,and the nasal tip was cosmetically pleasing.The result was satisfactory,and the subjective satisfaction scores were (3.4 ± 0.5).Among all patients,one experienced infection and two cases had septal hematoma.After timely postoperative management,a slight deformation of the cartilage scaffold occurred in 3 cases,and a secondary operation was performed after 6 months.All three patients showed obvious improvement and no severe complications occurred.Conclusion Application of autologous costal cartilage in rhinoplasty revision has certain clinical value.Complications can be controlled,and the results were reliable.
