Clinical application of the Step Nipple Retractor for correction of nipple invagination

Abstract：Objective To observe the clinical etficacy of the step nipple retractor in the treatment of nipple invagination.Methods From June 2013 to January 2017,22 patients with inverted nipple were treatment with self-designed adjustable step nipple retractor.The self-designed adjustable step nipple retractor was divided into 3 gears (3 mm of gear distance).After the inverted nipple was released during the operation,the mammary areola was pulled out.A rod with a diameter of 2 mm was crossed through the nipple base and fixed to the traction ring.After surgery,the pull rod was adjusted and raised one gear per month.According to the degree of severity of nipple invagination,traction was completed at between 3 to 4 months.After maintainance for 3 to 6 months,the traction was removed.Results This method was used to correct 44 inverted nipples of 22 patients from June 2013 to January 2017.The follow-up period ranged from 6 to 18 months.No complications such as infection,depigmentation of skin,sensory disturbance,or nipple necrosis occurred.All patients were satisfied with therapeutic effect and the aesthetic result of the surgery.Conclusion The step nipple retractor was a simple,safe,and reliable method for correction of inverted nipples.Satisfying aesthetic results can be achieved without destroying breasffeeding function or nipple sensation.
