Application of transfer transplantation of orbital fat flap combined with orbicularis oculi muscle flap in patients with severe depression of the upper eyelid who underwent double eyelid construction

Abstract：Objective To explore the results of the application of transfer transplantation of the orbital fat flap combined with the orbicularis oculi muscle flap in patients with severe upper eyelid depression who underwent double eyelid construction.Methods Below the double eyelid incision,the orbicularis oculi muscle and part of the anterior tarsal tissues were stripped and transferred to area above the double eyelid incision.The orbital fat was fully released,and the lateral orbital fat was transferred to the medial side.Results All 30 patients were satisfied with the postoperative results.Twenty-one patients were followed-up for 24 months.The incisions healed well,and no obvious scar,sags,or crests were present in the upper eyelid.Conclusion For patients with severe upper eyelid depression,a technique employing the orbital fat flap combined with the orbicularis oculi muscle flap during the construction of the double eyelid yields good results,a natural appearance,and a single incision for the double eyelid.It can also solve the problems of insufficient transferable fat flap tissue and the inability of the orbicularis oculi muscle flap to reach the bottom of the depression.
