Prevention and analysis of the complications of post-auricular soft tissue expander implantation for congenital microtia

Abstract:Objective To investigate the prevention of complications of post-auricular soft tissue expander implantation for congenital microtia.Methods A total of 418 patients with congenital microtia malformations were retrospectively analyzed from January to December 2015.A total of 428 soft tissue expanders were implanted in the 418 patients.Complications included hematoma (n=7),infection (n=8),wound dehiscence (n=5),flap necrosis (n=1 1),and facial nerve injury (n=2).All cases with complications received symptomatic treatment.Results Dilator water injection and stage Ⅱ operations for 7 patients with hematoma and 8 patients with infection were unaffected after removing the hematoma and controlling infections.In five patients,wound dehiscence occurred.Two of these cases were treated with debridement and suture,the remaining cases underwent stage Ⅱ operations.The expander was removed for 1 patient with flap necrosis,and stage Ⅱ operations were carried out immediately for the remainder of the patients.There was no obvious recovery after neurotrophic drug treatment for 2 cases with facial nerve injury.Conclusion Standardized operation procedure,careful observation of local adverse reactions and timely treatment during the expander dilator water injection can effectively reduce the incidence of complications and ensure a smooth stage Ⅱ operation.
