Abstract:Objective To explore the method and efficacy of autologous auricular cartilage-free transplantation in the correction of severe crater nipple. Methods From July 2012 to July 2015,12 cases (21 sides) with severe crater nipples were involved in this study.Four symmetric and equal rhombus regions were designed in the mammary areola. The areola skin was removed within the rhombus regions.Contracture fiber bundle and smooth muscle under the nipple in the rhombus region within the lower quadrant was thoroughly released. An incision was made following the long diagonal, then the concha cartilage was trimmed into a cylindrical shape and transplanted to the nipple and the base of the nipple. The concha cartilage was encased in mammary tissue under the areola.Purse string suture was performed to fix the position of cylinder cartilage and the nipple base was tightened. Finally, the incisions were sutured layer by layer. Results All patients were followed up from 6 to 15 months. All incisions were primarily healed. No nipple necrosis or numbness occurred. The erection of the nipple was normal. The nipple convexity and appearance was satisfactory. The tactile impression had the unique tenacity of papillary tissue. The incision scar was not obvious and there was no recurrence. Conclusion Autologous auricular cartilage free transplantation can be used as one of the effective methods to correct severe crater nipple.
