Application of modified keystone perforator island flap in the repair of full-thickness scalp defects

Abstract:Objective To explore the approach and efficacy of the keystone perforator island flap in the repair of full-thickness scalp defects.Methods From June 2014 to June 2016,10 patients with full-thickness scalp defects were admitted to the hospital.The wound areas ranged from 3.0 cm × 2.0 cm to 6.0 cm × 5.0 cm,and the flap cutting size ranged from 4.0 cm × 2.5 cm to 8.0 cm× 5.0 cm.The keystone perforator island flap was used in all patients.In order to reduce the influence of hair follicle injury and hair growth direction,the flap was designed along the edge of the defect and the head vessel was taken as the pedicle.This was done to relieve suture tension,but also plays a role in achieving good cosmetic results.Results All flaps survived during follow up from 1 to 6 months.No fat,sunken tissue,infection,or necrosis occurred.All flaps achieved an aesthetically natural appearance.Conclusion The head keystone perforator island flap was characterized by dense perforator vessels,rich blood supply,and large scope of nutritional skin and subcutaneous tissue.It provides greater efficacy in cases of small to medium-sized full thickness scalp defects.
