Application of the trans-conjunctival approach combined with the finger separation technique in removing cavernous hemangioma within the orbital muscle cone

Abstract:Objective To investigate the clinical efficacy of the removal of cavernous hemangiomas within the orbital muscle cone via the surgical trans-conjunctival pathway and finger separation.Methods Forty-seven patients with unilateral cavernous hemangiomas in the orbital muscle cone were recruited.According to the location of the tumor and optic nerve,the patients were divided into two groups:tumors in the lateral group were located lateral to the optic nerve (33 cases,34 tumors) and tumors in the inner group were located inside the optic nerve (14 cases,15 tumors).In each case,the tumor was probed by the little finger to ensure its location.After that,the surgeon inserted his/her little finger into the orbit along the tumor surface to achieve the final blunt separation until the tumor was totally separated from its neighboring tissues,and the tumor was successfully revealed.Results All cavernous hemangiomas were completely and successfully removed during a single surgeryin 47 cases,and the surgical duration was 20 to 30 minutes.In addition,operation time between the two groups was obviously different.It took less time to remove the tumor in the lateral group (P<0.05).Varying levels of transient diplopia were demonstrated among all the patients post-operation.Moreover,the proportion of diplopia showed dramatic difference between the two groups during the follow up.The duration of recovery in the lateral group was less than in the inner group.The follow-up lasted from 3 months to 2 years.None of the patients experienced blindness,eye ball restraint,incision malformation of angular skin,obvious scars,relapse or infection.Conclusion The combination of the trans-conjunctival approach combined with finger separation in cavernous hemangioma removal decreased the rate of lateral orbitotomy.No obvious scars occurred.The operation and recovery time was shorter for the cavernous hemangioma outside the optic nerve,and the patients' satisfaction was high.It is more suitable for beginners.
