Abstract：Objective　To explore the cause of secondary lower eyelid bag and try to find a better plastic operation. Methods　On sagital plane: the skin and muscular layers were resected and fastened separately. Then herniated orbital fat was partially removed or released. On coronary plane: the skin was resected and fastened on horizontal and vertical directions. The orbiculus muscle was resected radially. Results　The results of 63 cases were satisfactory. Conclusion　The stereoscopic multi-directional plastic operation of lower eyelid bag is a good procedure for secondary lower eyelid bag.
