Predictive value of combined detection of serum D-dimer,SAA and NT-proBNP for COPD combined with acute lower respiratory tract infection

Abstract:Objective To explore the predictive value of combined detection of D-dimer,serum amyloid A(SAA)and N-terminal pro-brain natriuretic peptide(NT-proBNP)for chronic obstructive pulmonary disease(COPD)combined with acute lower respiratory tract infection(ALRTI).Methods The 87 patients with COPD admitted to the Third Affiliated Hospital of Xuzhou Medical University from January 2024 to July 2025 were selected as research subjects,and were divided into comorbidity group(30 cases)and non-comorbidity group(57 cases)based on whether they experienced ALRTI.Binary Logistic regression analysis was used to investigate the influencing factors of COPD with ALRTI,and its prediction model was constructed.The receiver operator characteristic curve(ROC curve)was plotted and the area under ROC curve(AUC)was calculated.The predictive values of D-dimer,SAA,NT-proBNP alone and in combination for COPD with ALRTI were analyzed.Results D-dimer,SAA,homocysteine(HCY)and NT-proBNP were independent risk factors for COPD with ALRTI,with odds ratios(OR)of 3 026.595[95%confidence interval(95%CI)of 2.480-3 694 404.721],10.004(95%CI of 1.639-61.058),1.843(95%CI of 1.097-3.098)and 17.895(95%CI of 1.493-214.526)and P values of 0.027,0.013,0.021 and 0.023,respectively.The AUC of combined detection of D-dimer,SAA and NT-proBNP for prediction of COPD with ALRTI was 0.957(95%CI of 0.912-1.000),with sensitivity and specificity of 98.2%and 86.7%,respectively.The AUC of predictive model was 0.977(95%CI of 0.934-1.000),with sensitivity and specificity of 98.2%and 96.7%,respectively.Conclusion The individual and combined detection of D-dimer,SAA and NT-proBNP,as well as predictive model,had predictive values for COPD with ALRTI,and the predictive performance of the combined detection of three indicators was superior to that of each individual indicator.
