Clinical value of combined detection of D-dimer,uric acid,triacylglycerol and cystatin C in early diagnosis of preeclampsia

Abstract：Objective To investigate the early predictive value of combined detection of D-dimer,uric acid(UA),triacylglycerol(TG)and cystatin C(CysC)for preeclampsia(PE).Methods The 200 pregnant women admitted to Xuzhou Heping Hospital from January 2021 to December 2024 were enrolled and divided into severe PE group(40 cases)and mild PE group(60 cases)based on disease severity.The 100 healthy pregnant women during the same period were included in control group.The level of D-dimer was measured by immunoturbidimetry,the levels of UA,TG and CysC were measured by enzymatic method,and the differences in levels of above indicators between two groups were compared.Risk factors for early PE were identified using multiple Logistic regression analysis.Receiver operator characteristic curve(ROC curve)was drawn,and the area under ROC curve(AUC)was calculated to evaluate the diagnostic value of above indicators for PE.Results The levels of D-dimer,UA,TG and CysC in severe PE group were higher than those in mild PE group and control group[D-dimer(mg/L):3.82(2.90,4.75)vs.2.35(1.80,3.10),1.10(0.75,1.45);UA(μmol/L):458.20(390.00,530.00)vs.365.80(300.00,430.00),265.40(220.00,310.00);TG(mmol/L):4.27(3.50,5.05)vs.3.15(2.40,3.90),1.98(1.30,2.65);CysC(mg/L):1.95±0.52 vs.1.42±0.38,0.89±0.25;all P<0.05].Multivariate Logistic regression analysis revealed that D-dimer,UA,TG and CysC were all independent risk factors for PE.The odds ratios(OR)for severe PE were 3.824,2.677,2.243 and 2.055.The OR values for mild PE were 2.153,1.984,1.750 and 1.626.The combined detection of D-dimer,UA,TG and CysC demonstrated the optimal predictive efficacy for severe PE(AUC was 0.937,sensitivity was 88.7%,specificity was 90.3%),which was significantly superior to any single indicator.Conclusion Combined detection of D-dimer with metabolic and renal injury markers significantly enhances the early prediction and stratified diagnosis of PE,providing a quantitative basis for clinical intervention.
