The clinical significance of avidity of anti-TGAb and anti-TPOAb detection in thyroid function judg-ment of hashimoto's thyroiditis patients

Abstract:Objective To investigate the clinical value of anti-thyroglobulin antibody (anti-TGAb) and anti-thyroid peroxidase antibody (anti-TPOAb) avidity detection for the thyroid function judgment in the patients with hashimoto's thyroiditis(HT). Methods 52 cases patients with HT from February 2013 to August 2014 in our hospital were collected, and they were divided into thyroid dysfunction group (33 cases) and eu-thyroidism group (19 cases). Using urea as denaturant, the relative avidity index (RAI) of anti-TGAb and an-ti-TPOAb were obtained by quantitative ELISA. The relationship between RAI and the thyroid function were analyzed. Results The RAI of anti-TGAb and anti-TPOAb in thyroid dysfunction group were all higher than that of euthyroidism group, and the differences all had statistical significance (t=3.720, t=2.840, P=0.000, P=0.007). The cutoff value in evaluating thyroid function of RAI of anti-TGAb and anti-TPOAb were 39 and 35, area under receiver operating characteristic curve were 0.779 and 0.730, sensitivity were 84.8% and 81.6%, specificity were 63.2%and 64.1%, respectively. The higher RAI of both two antibodies which were dominant in thyroid dysfunction group was 54.5% (18/33), in euthyroidism group was 15.8% (3/19), and the difference had statistical significance (χ2=7.520, P=0.007). The lower RAI of both two antibodies which were dominant in thyroid dysfunction group was 6.1%(2/33), in euthyroidism group was 47.4%(9/19), and the difference had statistical significance (χ2=12.330, P=0.000). The higher RAI of only one kind antibody which was dominant in thyroid dysfunction group was 39.4%(13/33), in euthyroidism group was 36.8%(7/19), and the difference had no statistical significance (χ2=0.030, P=0.868). Conclusion The RAI of anti-TGAb and anti-TPOAb are associated with the thyroid function of HT patients, which can provide suggestion for the prognosis evaluation and clinical therapy.
