Analysis of the Therapeutic Effect of Total Hip Arthroplasty Combined with Greater Trochanteric Osteotomy in the Treatment of Severe Coxa Vara Deformity

Abstract:Objective To analyze the clinical efficacy of total hip arthroplasty combined with greater trochanteric osteotomy for the treatment of severe coxa vara deformity.Methods A retrospective analysis was conducted on 8 patients(10 hips)with severe coxa vara deformity treated at Beijing Jishuitan hospital from June 2018 to July 2022,including 2 males and 6 females,aged 35~69 years[mean(56.10±9.70)years].All patients underwent greater trochanteric osteotomy during surgery.The Wagner Cone stem was used on the femoral side to adjust prosthesis anteversion and limb length.After implantation of the hip prosthesis,a greater trochanteric plate was used for reduction and fixation of the greater trochanter.Evaluation indicators included the Harris hip score,Charnley Hip Score,visual analogue scale(VAS)for pain,improvement in limb length,healing of the greater trochanteric osteotomy,and complications.Results The mean operative time was(168.50±50.70)min,and the mean intraoperative blood loss was(810.00±287.92)mL.Postoperative follow-up ranged from 2.0 to 7.1 years,with a mean of(3.86±1.37)years.The affected limb was lengthened by an average of(29.40±4.27)mm postoperatively.The postoperative Harris hip score(84.50±3.85),Charnley score(13.80±0.87)and VAS(1.50±0.81)showed significant improvement compared to preoperative scores(P<0.05).At the final follow-up,all patients exhibited good healing of the greater trochanteric osteotomy,with no complications such as prosthesis loosening,dislocation,or internal fixation failure.Conclusion Total hip arthroplasty combined with greater trochanteric osteotomy is an effective treatment for severe coxa vara deformity.It significantly improves joint function and lengthens the limb,with a low risk of osteotomy displacement and nonunion,making it a feasible treatment option.
