Comparison of the Efficacy of Spinal Endoscopy Combined with Percutaneous Screws in the Treatment of Thoracolumbar Tuberculosis

Abstract:Objective To evaluate the clinical efficacy of endoscopic debridement combined with autologous iliac bone interbody fusion and percutaneous pedicle screw fixation in the treatment of thoracolumbar spinal tuberculosis.Methods A retrospective study was performed on 44 patients with thoracolumbar spinal tuberculosis treated at Liuzhou people's hospital from June 2020 to December 2023,including 24 males and 20 females,aged 20~77 years[mean(54.00±12.00)years].Patients were divided into an experimental group(n=22),treated with endoscope-assisted debridement combined with autologous iliac bone interbody fusion and percutaneous pedicle screw fixation,and a control group(n=22),treated with conventional posterior open surgery.Surgical parameters,visual analogue scale(VAS),Oswestry disability index(ODI),erythrocyte sedimentation rate(ESR),C-reactive protein(CRP),Cobb angle,and postoperative complications were compared.Results All patients were followed up for at least 12 months.The experimental group had significantly less intraoperative blood loss,shorter operative time,and shorter hospital stay than the control group(P<0.05).Postoperative VAS scores at 1,3,6,and 12 months and ODI scores at 1 and 6 months were significantly improved in the experimental group(P<0.05).The Cobb angle correction at 1,6,and 12 months and ESR and CRP levels at 6 and 12 months were also significantly better in the experimental group(P<0.05).In the experimental group,three cases of postoperative lower limb numbness occurred,with no secondary surgeries required.In the control group,3 cases of abscess and poor wound healing occurred(requiring secondary surgery),along with 2 cases of lower limb numbness and sensory decline,and 1 case of lower limb muscle weakness.Conclusion Endoscopic debridement combined with autologous iliac bone interbody fusion and percutaneous pedicle screw fixation is a minimally invasive,effective,and safe surgical approach for the treatment of thoracolumbar spinal tuberculosis.
