The Correlation of Serum sRANKL,MIP-1β,and CTRP3 with the Severity and Prognosis of Knee Osteoarthritis

Abstract:Objective To explore the correlations of serum soluble receptor activator of nuclear factor-κB ligand(sRANKL),macrophage inflammatory protein-1β(MIP-1β),and C1q/tumor necrosis factor-related protein-3(CTRP3)levels with the severity and postoperative prognosis of knee osteoarthritis(KOA).Methods From June 2022 to July 2024,106 patients with knee osteoarthritis who underwent arthroscopic surgery in Daqing Oilfield General Hospital were designated as disease group,including 56 males and 50 females;age 56~73 years old,mean age(65.95±6.89)years.They were classified into 2nd grade group(n=37),3rd grade group(n=44),and 4th grade group(n=25)complying with the severity of their condition.They were also classified into good prognosis group(n=74)and poor prognosis group(n=32)complying with the postoperative disease outcome.Meantime,106 healthy individuals who underwent physical examine were labeled as control group,including 53 males and 53 females;their ages were 53~72 years old,with a mean age of(65.02±6.77)years.After ELISA detection,the serum sRANKL,MIP-1β,CTRP3 and general clinical data were compared.Spearman rank correlation was used to discuss the correlation between serum sRANKL,MIP-1β,CTRP3 and severity of condition.Logistic regression was used to discuss the influencing factors of poor prognosis.Receiver operating characteristic(ROC)curves were plotted to analyze the predictive value of serum sRANKL,MIP-1β,and CTRP3 for disease outcomes.Results The disease group had obviously higher serum sRANKL and MIP-1β than the control group,and clearly lower CTRP3 than the control group(P<0.05).The 3rd and 4th grade groups had clearly higher serum sRANKL and MIP-1β,and obviously lower CTRP3 than the 2nd grade group(P<0.05),moreover,the 4th grade group had obviously higher serum sRANKL and MIP-1β,and obviously lower CTRP3 than the 3rd grade group(P<0.05).The sRANKL and MIP-1β were positively related to the severity of the condition(r=0.523,0.503,P<0.05),while CTRP3 was negatively related to the severity of the condition(r=-0.508,P<0.05).The poor prognosis group had clearly higher numbers of affected ventricles,sRANKL,and MIP-1β(P<0.05),and clearly lower CTRP3 than the good prognosis group(P<0.05).The combination of serum sRANKL,MIP-1β,and CTRP3 had an area under curve(AUC)of 0.962 for predicting patient prognosis,which was obviously better than the individual predictions of sRANKL(Z=2.532,P=0.011),MIP-1β(Z=2.595,P=0.010),and CTRP3(Z=2.950,P=0.003).Elevated levels of sRANKL and MIP-1β were risk factors for poor prognosis,while elevated level of CTRP3 was a protective factor for poor patient prognosis(P<0.05).Conclusion Serum sRANKL and MIP-1β levels are elevated in KOA patients,while CTRP3 levels are reduced,and these markers are closely correlated with disease severity and postoperative outcomes,suggesting their potential as biomarkers for assessing disease progression and prognosis in KOA.
