Application of ACL Reconstruction Combined with ALL Reconstruction in Anterior Cruciate Ligament Injury with High Posterior Tibial Slope Angle

Abstract:Objective To investigate the effectiveness of anterior cruciate ligament(ACL)combined with anterior lateral ligament(ALL)reconstruction in anterior cruciate ligament injury with high posterior tibial slope angle.Methods We included 67 patients with primary ACL rupture and PTSA≥15 ° treated at The First People's Hospital of Kunming from January 2021 to December 2022.The patients were divided into two groups:the experimental group(n=34)underwent combined ACL and ALL reconstruction,comprising 22 males and 12 females,with a mean age of(28.71±8.80)years;the control group(n=33)underwent isolated ACL reconstruction,comprising 22 males and 11 females,with a mean age of(32.82±9.55)years.We compared the differences between the affected and healthy sides under LIGS stress,knee joint pivot shift test,Lysholm score,International Knee Documentation Committee(IKDC)score,and Tegner score between the two groups preoperatively and at the final follow-up.Results All patients were followed up for at least 24 months postoperatively.At the final follow-up,the difference in affected-to-healthy side measurements under LIGS stress was(0.80±3.17)mm in the experimental group and(3.31±3.36)mm in the control group(P<0.05).The positive rate of the pivot shift test was 2.90%in the experimental group and 24.20%in the control group(P<0.05).The Lysholm score,IKDC score,and Tegner score were significantly higher in the experimental group compared to the control group(P<0.05).Conclusion For ACL ruptures with PTSA≥15 °,combining ACL reconstruction with ALL reconstruction provides a significant auxiliary enhancement effect and offers an effective treatment option for clinical practice.
