Treatment of Lower Limb Obliterative Vasculitis with Bone Transport Combined with Selective Sensory Neurectomy

Abstract:Objective To analyze the efficacy of tibial transverse transport(TTT)combined with selective sensory neurectomy(SSN)in the treatment of thromboangitis obliterans(TAO)of the lower extremities.Methods A retrospective analysis was conducted on 47 patients with TAO who underwent TTT combined with SSN treatment in Xinjiang Urumqi Autonomous Region People's Hospital from June 2018 to June 2024.The cohort included 30 males and 17 females,aged 45~71 years[mean age(52.5±3.5)years].The affected side was left in 23 cases,right in 21 cases,and bilateral in 3 cases.Disease duration ranged from 3 to 11 years[mean(3.5±1.5)years].Postoperative evaluations included wound healing,foot skin temperature,transcutaneous oxygen pressure(TcpO2),ankle-brachial index(ABI),intermittent claudication distance,and visual analogue scale(VAS)for pain.Results Patients were followed up for 6~12 months[mean(9.0±2.4)months].All foot ulcers healed completely.Two patients developed pin tract infections,which were resolved through skin incision and tension reduction and outpatient dressing changes.No recurrent foot ulcers or iatrogenic fractures occurred.At final follow-up,significant improvements were observed in foot skin temperature,TcpO2,ABI,intermittent claudication distance,and VAS scores compared to preoperative levels(P<0.05).Conclusion TTT combined with SSN is a minimally invasive and effective treatment for TAO,particularly in alleviating ischemic pain caused by vascular occlusion.With strict surgical indication screening,this combined approach holds clinical promotion value for TAO treatment.
