Comparison of the Efficacy of the Femoral Neck System Versus Cannulated Cancellous Screws in the Treatment of Femoral Neck Fracture in Different Age Groups

Abstract:Objective To compare the clinical outcomes of femoral neck system(FNS)versus cannulated cancellous screws(CCS)in treating Garden Ⅲ~Ⅳ femoral neck fractures across different age groups.Methods A retrospective analysis was performed on 81 patients[52 males and 29 females,aged 18 to 65 years,mean(53.83±10.53)years]with femoral neck fractures treated at the First Affiliated Hospital of Xinjiang Medical University from December 2019 to February 2023.Patients were divided into FNS group and CCS group,with further subdivision into FNS elderly,FNS non-elderly,CCS elderly,and CCS non-elderly subgroups.No statistically significant differences were found in gender,preoperative Garden classification,or Singh index among the four subgroups(P>0.05).Results Patients were followed up for 12 to 24 months[mean(14.00±4.25)months].No significant differences were observed in operation time,hospital stay,or postoperative complication rates among the four subgroups(P>0.05).The FNS non-elderly group demonstrated significantly higher postoperative Harris scores compared with the other three subgroups(P<0.05).The FNS group had significantly higher intraoperative blood loss than the CCS group(P<0.05),but fewer fluoroscopic images,shorter full-weight-bearing time,and lower postoperative complication rates(P<0.05).Postoperative Garden classification did not differ significantly between the FNS and CCS groups(P>0.05).Conclusion For non-elderly patients,FNS offers better outcomes than CCS in treating Garden Ⅲ~Ⅳ femoral neck fractures,characterized by fewer fluoroscopic images,shorter full-weight-bearing time,and improved hip function.In elderly patients,FNS and CCS achieve comparable hip functional recovery,but FNS results in fewer postoperative complications.
