Abstract:Unicompartmental knee arthroplasty(UKA)is recognized as an effective treatment for knee osteoarthritis(KOA),however, periprosthetic joint infection(PJI)remains one of the most serious complications following this procedure. With the increasing number of UKA surgeries in China, implementing effective preventive measures is crucial to reduce the incidence of PJI and enhance patients'quality of life.Despite this need, there is currently a lack of guidelines and expert consensus in China regarding the prevention of PJI after UKA.In response, the national orthopaedic medical center knee preservation alliance,in collaboration with the bone and joint protection and health branch of the Chinese association of geriatrics and healthcare, convened a panel of experts. These experts conducted a comprehensive review of the relevant literature, taking into account China's specific national conditions, and adhered to the principles of evidence-based medicine.Utilizing the Delphi method, they developed a consensus through several rounds of correspondence and conference reviews. This consensus encompasses 16 recommendations for preoperative assessment, intraoperative optimization, and postoperative management.Specifically, the preoperative phase includes evaluating and improving nutritional status, adjusting blood glucose levels, and controlling body mass index.During the intraoperative phase, the consensus advises the rational use of tourniquets, irrigation solutions, and appropriate antibiotics. In the postoperative phase, the focus is on the prophylactic use of antibiotics during drain placement and other invasive procedures following UKA,aiming to minimize the risk of PJI.
