The Results of Direct Anterior Approach for Primary Total Hip Arthroplasty

Abstract:Objective To summarize the clinical and radiographic outcomes of direct anterior approach for total hip ar-throplasty(THA). Methods From January 2010 to January 2013,85 patients with hip joint disease were treated by THA u-sing the direct anterior approach. The operative time,blood loss,length of skin incision,length of hospital stay,cup inclination, cup anteversion,stem orientation,the level of activity(UCLA score),Harris Hip Score(HHS),and complications were recor-ded. Results The operative time was 55 to 105(71. 3 ± 15. 4)min. The blood loss was 300 to 650(393. 2 ± 189. 5)mL. The length of skin incision was 8 to 13(11. 2 ± 2. 4)cm. The length of hospital stay was 5 to 12(7. 6 ± 1. 3)days. The cup incli-nation and anteversion was 35° ~ 55°(46. 3 ± 5. 9)° and 5° ~ 25°(16. 4 ± 9. 7)°respectively;The rate neutral stem orientation was 94. 1% ;The mean level of activity(UCLA score)improved from 3. 8 ± 2. 1 points preoperatively to(6. 4 ± 1. 6)points at final follow-up(P ﹤ 0. 001);The mean HHS improved from(56. 7 ± 12. 4)points preoperatively to(96. 5 ± 8. 2)points at final follow-up(P ﹤ 0. 001). The overall rate of complication was 10. 6% ,which were mild,without required secondary surgi-cal treatment. Conclusion The direct anterior approach for total hip arthroplasty can achieve a satisfactory clinical and radio-graphic outcomes with minor complications.
