Cannulated Locking Plate and Tension Screw for Femoral Neck Fracture in the Middle-aged and Young
Abstract:Objective To discuss the selection of cannulated locking plate or cannulated tension screw in treatment of femoral neck fracture in the middle-aged and young.Methods From January 2007 to December 2012,89 cases of femoral neck fractures were divided two groups,one group of 42 cases underwent cannulated locking plates,the other 47 cases under-went cannulated tension screws. The patients aged 17 to 60 years with an average of 48. 7 years. We compared the treatment effect of the two groups respectively in the stable type and unstable type.( The definition of stable type is the angle of Pauwels less than 30 degree. )Long-term follow-up of the hip joint function was evaluated according to Harris score system. Results This group of 89 patients were followed up fo r26~48 months(38 months on average).Therate of good and excellent of cannulated locking plate in the stable femoral neck fracture group was 78. 1%,86. 4% in the cannulated tension screw group. Hollow nail was better than that of locking plate group by chi-square test(P<0. 05),and the fracture healing time,inci-dence of bone nonunion,femoral head necrosis rate was lower than that of locking plate group. However,in the non stable frac-tures group,the cannulated locking plate was 73. 9%,the cannulated tension screw group was 64. 0%,P<0. 05. Conclusion For the young and middle-aged femoral fracture,anatomical reduction and internal fixation treatment should be taken for.If it is stable femoral neck fracture,cannulated tension screw should be adopted. For unstable fracture,or patients with osteoporo-sis should consider proximal femoral locking plate.
