Multivariable Logistic Regression Anlysis of Fuctors Related to Curative Effect in Open Pilion Frature
Abstract:Objective To explore factors related to efficacy of surgical operation of Open Pilon fractures. Methods A to-tal of 106 patients of open Pilon fractures were treated by surgical Operationin ourdepartment between March 2006 and March 2013. Their data were retrospectively analyzed. According to clinical information and follow-ups,we selected factors such as gender,age,Rüedi-Allgower type of fracture,preoperativeskeletal traction,CT of ankle joint,tension vesicle of fracture site,soft-tissue injury,swelling of injury limb,Injury of blood vessel,nerve injury,surgical options,complicated with fractures of fibula, complicated with diabetes,complicated with osteoporosis,postoperative complications and partial weight bearing time. which may affect the excellent and good rate of ankle after surgical operation for treating open Pilon fractures. The selectedfactors were then grouped and assigned,after unrelated factors wereexcluded by one-way χ2 analysis,multivariableLogistic regression analy-sis was performed. Results The involved 106 patients were followed up for 6 ~ 18 months. According to Mazur assessment cri-teria,ankle function was excellent in 41 cases,good in 32 cases,fair in 19 cases,and poor in 14 cases atthe final follow-up,the excellent and good rate was 69% . Radiographic results showed that,the patients were divided into two groups according to the presence of the displacement,Rüedi-AllgowerⅠ(n = 13)、Rüedi-AllgowerⅡ(n = 48)as a group,and Rüedi-AllgowerⅢ(n =45)as the other group,the excellent and good rate of ankle was 93% and 36% ,respectively. In serious andslight groups of soft tissue injury,the excellent and good rate of ankle was 28% and 90% ,respectively. In open reduction and internal fixation (ORIF)groups and limited internal fixation combined with external fixation groups,the excellent and good rate of ankle was 28% and 84% ,respectively. In the complicated with osteoporosis groups and the normal groups,the excellent and good rate of ankle was 33% and 85% ,respectively. In thecomplication and non-complication groups,the excellent and good rate of ankle was 76% and 21% ,respectively. These factor groupsshowed significant differences(P < 0. 05). Conclusion Multiariable Lo-gistic regression analysis showed that,Rüedi-Allgower type of fracture,soft tissue injury,surgical options,complicated with oste-oporosis,postoperative complications are the five risk factors for surgical operation of open Pilon fractures.
