Hip Arthroplasty for Failed Internal Fixation of Intertrochanteric Fractures

Abstract:Objective To investigate the result of Hip arthroplasty for failed internal fixation of intertrochanteric frac-tures. Methods 21 cases who had hip arthroplasty after internal fixation failure of intertrochanteric fractures were reviewed. There were 15 males and 6 females. The mean age was 62. 8 years(range,40 ~ 79 years)at the time of the fracture. The aver-age interval from fracture to arthroplasty was 11. 4 months(range,5 ~ 26 months). Results The prosthesis types included 20 total hip arthroplasties and 1 bipolar hip arthroplasties. The Classic femoral stem of Link Company of Germany was used in ev-ery patient. Although for some cases,the stem length did not extend well beyond the most distal screw holes,there was no stress fracture happened. There were no complications such as intraoperative fracture of the greater trochanter and postoperative dislo-cation. The Harris Hip Score significantly improved from 43. 2 points before the conversion to 84. 5 points after an average fol-low - up of 15. 6 months(range,5 ~ 48 months). There was no revision for mechanical failure. Conclusion The effect of the hip replacement for patients with failed internal fixation of intertrochanteric fractures was confirmed. This method can shorten the time on the bed and reduce the complications. It benefits the earlier functional recovery,but we must control operation indi-cation. The long term efficacy is necessary to be further observed.
