Comparison of laparoscopic mesh implantation and autologous tissue repair for severe pelvic organ prolapse

Abstract:Objective:To explore the efficacy and safety of laparoscopic mesh implantation(LMI)and autologous tissue repair(ATR)in the treatment of severe pelvic organ prolapse,and to provide clinical basis for optimizing the surgical plan.Methods:All 120 patients with severe pelvic organ prolapse treated in Puyang Oilfield General Hospital from January 2020 to December 2024 were randomly divided into ATR group(60 cases)and LMI group(60 cases),and the efficacy and safety of the two operations were compared in terms of anatomical reduction success rate,functional im-provement and postoperative complications.Results:Six months after operation,the success rate of anatomical reduction in LMI group(96.7%)was significantly higher than that in ATR group(87.0%)(P<0.05).The scores of interna-tional urinary incontinence advisory committee questionnaire(ICIQ-SF)and dysdefecation scale(CSS)in LMI group were significantly lower than those in ATR group(P<0.05).There was no significant difference between the two groups in the scores of female sexual function index scale(FSFI)and the incidence of infection,hematoma formation and deep vein thrombosis(DVT)(P>0.05),but the proportion of visual analogue scale(VAS)≥4in LMI group was higher(P<0.05).Conclusion:Laparoscopic mesh implantation provides superior anatomical and functional outcomes compared to autologous tissue repair,but is associated with a higher incidence of postoperative pain.Autologous tissue repair,with no foreign material implantation,offers more postoperative comfort.Individualized treatment plans should be formulated based on patient-specific factors to optimize surgical outcomes.
