Comparative study on the effect of minimally invasive drilling and drainage,minimally invasive small bone window craniotomy and microsurgical hematoma evacuation for cerebral hemorrhage

Abstract:Objective:To compare the clinical outcomes of minimally invasive burr hole drainage and minimally inva-sive microcraniotomy with microendoscopic hematoma evacuation in patients with intracerebral hemorrhage.Methods:From October 2023 to December 2024,62 patients with cerebral hemorrhage were randomly divided into observation group(n=31)and control group(n=31)according to the software lottery method.Patients in the control group un-derwent minimally invasive small bone window craniotomy with microscopic hematoma evacuation,while those in the observation group underwent minimally invasive drilling and drainage.The residual hematoma rate,rebleeding rate,mortality rate,postoperative activities of daily living(ADL)classification,complication rate,operation time,intraopera-tive blood loss,and hospital stay were compared between the two groups.Results:There was no significant difference in the residual rate of hematoma,mortality,ADL classification and incidence of complications between the two groups(P>0.05).The rebleeding rate in the control group was lower than that in the observation group,and the operation time,blood loss and hospitalization time in the observation group were less than those in the control group,with statistical significance(P<0.05).Conclusion:Minimally invasive drilling and drainage and minimally invasive small bone window craniotomy under microscope are used to treat patients with cerebral hemorrhage,and the mortality and complications are similar,which can effectively remove hematoma and improve the ability of daily living.Minimally invasive small bone window craniotomy under microscope can reduce the rate of rebleeding,and minimally invasive drilling and drain-age can reduce the amount of blood loss during operation and shorten the operation time and hospitalization time.
