A real-world study of pegylated recombinant human granulocyte colony-stimulating factor for sec-ondary prevention of chemotherapy-induced neutropenia

Abstract:Objective:To find the optimal prophylactic dose of pegylated recombinant human granulocyte colony-stimulating factor(PEG-rhG-CSF)injection by comparing the efficacy,safety,and economicity of its different doses(3 mg and 6 mg)for the secondary prevention with an intermediate risk of chemotherapy-induced neutropenia.Meth-ods:From January 2023 to January 2025,398 patients with malignant solid tumor who received chemotherapy in Jincheng People's Hospital and used pegylated recombinant human granulocyte stimulating factor injection for secondary prevention of neutropenia or deficiency were selected and divided into two groups:3 mg(n=169)and 6 mg(n=229).The effectiveness,safety and economy of the two groups in preventing neutropenia and deficiency were compared by SPSS 24.0 software.Results:The overall incidence of neutropenia in the 3 mg and 6 mg groups was 11.83%(20/169)and 9.61%(22/229)respectively,with no statistical significance(P>0.05).The incidence of grade 3 and grade 4 granulocytopenia in 6 mg group was lower than that in 3 mg group,which were 3.49%(8/229)and 3.55%(6/169)respectively,but the difference was not statistically significant(P>0.05).There were no serious adverse reactions in the two groups,and the incidence of general adverse reactions such as bone pain and fever was similar in the two groups,which were 2.37%(4/169)and 2.62%(6/229)respectively,with no statistical significance(P>0.05).The direct economic cost(680 yuan)of the 3 mg group was lower than that of the 6 mg group(1360 yuan),and the differ-ence between the two groups was statistically significant(P<0.05).The ratio of single treatment expense to hospitali-zation expense in 3 mg group was(12.89±3.95)%,which was lower than that in 6 mg group(16.15±6.28)%,but there was no statistical difference between the two groups(P>0.05).Conclusion:For the secondary prevention of neu-tropenia or deficiency in non-obese patients(with BMI in the normal range)receiving chemotherapy regimens with an intermediate risk of chemotherapy-induced neutropenia,the effectand safety of the 3mg dose of PEG-rhG-CSF was similar to the 6mg dose,while be more economical.
