A case of small intestinal obstruction with small intestinal fistula caused by small intestine metastasis of lung cancer

Abstract:Objective:To analyze the clinical characteristics,diagnosis and treatment process,and prognosis of a pa-tient with intestinal obstruction combined with small intestinal fistula caused by small intestine metastasis of lung canc-er,in order to accumulate experience in the diagnosis and treatment of this disease.Methods:Clinical data of a patient with intestinal obstruction combined with small intestinal fistula caused by small intestine metastasis of lung cancer were collected,and literature review and study were conducted.Results:The patient was admitted to hospital due to'chest tightness and high fever,accompanied by nausea and vomiting after eating for more than one day'.The patient had a previous diagnosis of lung squamous cell carcinoma.The initial diagnosis was acute intestinal obstruction combined with acute peritonitis.After emergency exploratory laparotomy,the patient was discharged.The diagnosis was corrected based on preoperative and postoperative pathology as:infiltrative squamous cell carcinoma of the lower lobe of the left lung,small intestinal squamous cell carcinoma,small intestinal fistula,acute peritonitis,and small intestinal obstruction.Conclusion:Acute conditions of intestinal obstruction combined with small intestinal fistula caused by small intestine metastasis of lung cancer are extremely rare clinically.When patients with lung cancer present with abdominal symp-toms,intestinal metastasis should be considered,indicating hematogenous spread and poor prognosis.Early correspond-ing treatment measures should be taken to relieve patient suffering and reduce medical burden.
