Analysis of interfering factors of transfusion compatibility testing in multiple myeloma patients and stratified treatment strategies

Abstract:Objective:To explore the main interfering factors of pre-transfusion compatibility testing in multiple myeloma(MM)patients and stratified treatment strategies.Methods:The data of 201 transfusion compatibility tests of 36 MM patients who received treatment in the people's hospital of chuxiong yi autonomous prefecture from September 2022 to December 2024 were included,and the tests were performed using a fully automated blood typing instrument and a blood-matching instrument,and the abnormal results(n=99)were analyzed by the test-tube method of re-peated testing,the saline replacement method,the 37℃and 4℃treatments,the coagulant amine method,the 2-mer-captoethanol(2-Me)pretreatment method,and irregular antibody specificity identification method to deal with inter-fering factors in a hierarchical manner so as to complete the pre-transfusion related tests.Results:CD38 monoantibody interference accounted for 61.62%(61/99),followed by blood group antibody attenuation[18.18%(18/99)]and pro-tein agglutination[12.12%(12/99)].The success rate of blood matching was significantly improved after treatment with coagulant amine method and 2-Me.Conclusion:There are many interfering factors in transfusion compatibility testing in MM patients,and there are many challenges in blood matching,among which CD38 monoclonal antibody is the main factor of interference.The establishment of standardized procedures and stratified processing in the transfusion department can improve the efficiency of transfusion compatibility testing and the safety of blood transfusion.
