Impact of corona virus disease-19 infection on clinical test indexes and prognosis of hematological tumor patients

Abstract:Objective:To analyse the effect of corona virus disease-19(COVID-19)on clinical test indexes and prognosis of haematological oncology patients.Methods:The clinical data of 101 patients with novel coronavirus infec-tion admitted to the Department of Haematology of the First Affiliated Hospital of Xi'an Jiaotong University from De-cember 2022 to February 2023 were retrospectively analyzed,and they were divided into transplantation group(25 ca-ses)and the non-transplantation group(76 cases)according to whether hematopoietic stem cell transplation.Labora-tory parameters and clinical outcomes were compared between the two groups before infection and at 1 week post-in-fection,>1 week post-infection,and among patients who cleared the infection,within each group.Results:At 1 week post-infection,the transplant group exhibited higher neutrophil(NE)counts than the non-transplant group(P<0.05),while D-dimer(D-D)levels were lower in the transplant group(P<0.05).After>1 week of infection,the trans-plant group had higher lymphocyte(LYM)counts than the non-transplant group(P<0.05),while fibrinogen(FIB)and interleukin-6(IL-6)levels were lower in the transplant group(P<0.05).After nucleic acid clearance,the non-transplant group had higher platelet(PLT)counts than the transplant group(P<0.05).Compared with before no-vel coronavirus infection,LYM and PLT decreased,and FIB,C-reactive protein(CRP)and IL-6 increased in the transplanted group after infection,with statistical significance(P<0.05).White blood cells(WBC),LYM,NE and PLT decreased,while FIB,D-D and CRP increased in the non-transplanted group,with statistical significance(P<0.05).The mortality rate of transplantation group was 12.00%(3/25),which was slightly higher than that of non-transplantation group(11.84%,9/76),but there was no statistical difference between the two groups(P=0.983).There were 16 cases(72.73%,16/22)in transplantation group and 37 cases(68.52%,37/54)in non-transplantation group,and there was no statistical difference between the two groups(χ2=0.389,P=0.533).Conclusion:corona virus disease-19 have a greater impact on laboratory markers and clinical outcomes in haematological oncology patients,with transplanted patients having a higher mortality rate than non-transplanted patients and transplanted patients having a longer duration of neo-coronavirus infections.
