The application of two kinds of lithotripsy combined with primary suture in patients with common bile duct stones

Abstract：Objective:To compare the curative effects of two kinds of lithotomy combined with primary suture on common bile duct stones.Methods:From April 2023 to December 2024,89 patients with choledocholithiasis were ran-domly divided into observation group(n=45)and control group(n=44)according to the numerical table method.The observation group was treated with mini-incision lithotripsy combined with primary suture at the junction of common bile duct through cystic duct,while the control group was treated with common bile duct exploration lithotomy com-bined with primary suture.The postoperative recovery,liver function index,stress response and complications of the two groups were compared.Results:The recovery time of gastrointestinal function,extubation time and hospitalization time in the observation group were shorter than those in the control group(P＜0.05).Three days after operation,the levels of alanine amino transfer(ALT),aspartate amino transfer(AST)and total bilirubin(TBiL)in the two groups were lower than those before operation,the observation group was lower than the control group,the difference was sta-tistically significant(P＜0.05).Three days after operation,the cortisol(Cor),epidephrine(E)and adrenocorticotropic hormone(ACTH)in the two groups were higher than those before operation,but the observation group was lower than that in the control group,with statistical significance(P＜0.05).After 3 months follow-up,the incidence of complications in the observation group was lower than that in the control group,and the difference was statistically significant(P＜0.05).Conclusion:Compared with the exploration of the common bile duct,microincision and stone removal through the con-fluence of the common bile duct are more conducive to postoperative recovery,with milder stress response and fewer complications in the treatment of patients with common bile duct stones.
