Predictive effect of lipid levels on clinical response to abesilide combined with fluvestrant treatment for HR+/HER2-type advanced breast cancer

Abstract:Objective:To explore the role of blood lipid levels in predicting the efficacy of the cyclin-dependent ki-nase 4/6(CDK4/6)inhibitor abemaciclib combined with fulvestrant in the treatment of patients with hormone receptor-positive and human epidermal growth factor receptor 2-negative(HR+/HER2-)advanced breast cancer.Methods:A total of 80 patients with HR+/HER2-type advanced breast cancer were treated with CDK4/6 inhibitor abexil and fulvestrant in Jiujiang First People's Hospital from January 2023 to December 2023.According to whether treatment ef-ficacy resistance occurred,they were divided into sensitive group and resistant group.The risk factors of blood lipid lev-els affecting the treatment effect of patients with HR+/HER2-advanced breast cancer were analyzed.A risk prediction model of blood lipid levels on the treatment effect of patients with HR+/HER2-advanced breast cancer was construc-ted,and the efficacy of the model was tested by the receiver operating characteristic curve(ROC).Results:The results of univariate analysis showed that total cholesterol(TC),triglyceride(TG),high-density lipoprotein cholesterol(HDL-C),and low-density lipoprotein cholesterol(LDL-C)were influencing factors for treatment efficacy resist-ance in patients with HR+/HER2-advanced breast cancer treated with the CDK4/6 inhibitor abemaciclib and fulves-trant,and the differences were statistically significant(P<0.05).The results of multivariate analysis showed that TC,TG,LDL-C,etc.were independent risk factors affecting the efficacy of patients with HR+/HER2-advanced breast cancer,and HDL-C was a protective factor,and the differences were statistically significant(P<0.05).The area un-der curve(AUC)of the predictive model was 0.898,the sensitivity was 0.778,and the specificity was 0.887,indicating that the model had good predictive power.Conclusion:The blood lipid levels of TC,TG,and LDL-C are independent risk factors affecting the efficacy of patients with HR+/HER2-advanced breast cancer,and HDL-C is a protective factor.The established model has good predictive efficacy.
