The value of NLR and DWI and ADC parameters in the assessment of haemorrhagic transformation in patients with acute cerebral infarction

Abstract:Objective:To investigate the value of neutrophil-to-lymphocyte ratio(NLR),magnetic resonance dif-fusion-weighted imaging(DWI)signal intensity,and apparent diffusion coefficient(ADC)parameters in the assess-ment of haemorrhagic transformation in patients with acute cerebral infarction.Methods:All 130 patients with acute cerebral infarction admitted to the first affiliated hospital of gannan medical university from December 2022 to January 2024 were selected as the study subjects,and they were divided into the haemorrhagic group and the non-haemorrhagic group according to whether the haemorrhagic transformation occurred or not.The clinical data,DWI signal intensity,ADC value and NLR of the two groups were compared,and the statistically significant factors were analyzed by Logistic regression to analyze the influencing factors of hemorrhagic transformation of acute cerebral infarction.ROC curve anal-ysis of DWI signal intensity value,ADC value and NLR for predicting hemorrhagic transformation of acute cerebral in-farction.Results:DWI signal intensity values and NLR were higher in the haemorrhage group than in the non-haemor-rhage group(P<0.05).ADC values were lower in the haemorrhagic group than in the non-haemorrhagic group(P<0.05).DWI signal strength(OR=7.582,95%CI1.655~9.558),ADC value(OR=2.867,95%CI 1.185~6.939)and NLR(OR=3.016,95%CI 1.949~4.666)were independent risk factors for haemorrhagic transformation of acute cer-ebral infarction(P<0.05).DWI signal intensity,ADC value and NLR combined detection predicted AUC value of hae-morrhagic transformation of acute cerebral infarction higher than that of single detection(ZDwI signal intensity~combined detection=5.055,ZADC value~combined detection 1.980,ZNLR~combined detection 4.422,P<0.05).Conclusion The combined detection of DWI signal intensity,ADC value and NLR can improve the diagnostic value of haemorrhagic transformation of acute cerebral infarction.
