Effect of balloon dilation diameter on carotid stent implantation in patients with symptomatic severe carotid artery stenosis

Abstract:Objective:To investigate the effect of different balloon dilation diameters on stent implantation in pa-tients with symptomatic severe carotid artery stenosis.Methods:From January 2019 to December 2023,the data of 136 patients who received symptomatic severe carotid artery stenosis in Neixiang County People's Hospital were retrospec-tively analyzed.Patients were divided into different groups according to the size of the pre—dilated balloon used in ca-rotid stent implantation.The control group(n=65)used a 4 mm×30 mm balloon,and the observation group(n=71)used a 5 mm×30 mm balloon.The surgical success rate of the two groups was analyzed.The changes of blood pres-sure,heart rate,peak time of contrast agent,relative peak time,stenosis degree,internal carotid artery diameter,maxi-mum systolic velocity and end—diastolic velocity and national institute of health stroke scale(NIHSS)and incidence of postoperative complications were compared between the two groups before and after surgery.The differences in postop-erative complications between the two groups were also evaluated.Results:Stents were successfully implanted in both groups,and the success rate of operation was 100%.The postoperative systolic blood pressure,diastolic blood pressure,heart rate and NIHSS score of the two groups were significantly lower than those before operation(P<0.05),but there was no statistical difference between the two groups(P>0.05).The postoperative stenosis rate,peak systolic ve-locity and end diastolic velocity of the two groups were lower than those before operation,and the observation group was significantly lower than that of the control group,with statistical significance(P<0.05).The levels of von wille-brand factor,endothelin and internal carotid artery diameter in the two groups were significantly higher than those be-fore operation(P<0.05),but there was no significant difference between the two groups(P>0.05).There was no significant difference in the incidence of postoperative complications between the two groups(P>0.05).Conclusion:Both 4 mm and 5 mm diameter balloons show good safety and efficacy in the treatment of symptomatic severe carotid artery stenosis.However,5 mm balloon pre—dilation showed a much stronger advantage in improving hemodynamics.
