Predictive value of geriatric nutritional risk index on postoperative complications in patients with spinal cord injury

Abstract:Objective:To investigate the predictive value of geriatric nutritional risk index(GNRI)on the risk of postoperative complications in patients with spinal cord injury(SCI).Methods:A retrospective analysis of the clinical data of 205 elderly patients with SCI who were admitted to and operated on by the spinal surgery of a tertiary hospital from January 2019 to December 2022,grouped according to whether complications occurred after surgery,and calculate the GNRI based on the results of the patient's preoperative laboratory examination,through univariate analysis and mul-tivariate logistic regression analysis the influencing factors of postoperative complications in SCI patients.Meanwhile,use receiver operating characteristic curve(ROC)to evaluate the predictive value of GNRI for postoperative complica-tions in SCI patients,and grouped according to GNRI critical value to compare the differences of hospitalization costs and length of stay in different groups of elderly SCI patients.Results:Among the 205 elderly patients with SCI,83 pa-tients(40.5%)experienced postoperative complications.The results of multivariate logistic regression analysis showed that spinal cord injury degree(ASIA)classification and GNRI were independent risk factors for postoperative complica-tions in elderly SCI patients(OR=14.537 and 0.843,both P<0.05).GNRI has a predictive effect on postoperative complications in elderly SCI patients,and the optimal critical value to distinguish the occurrence of complications is 96.1,and the area under the ROC curve is 0.774.The hospitalization costs and length of stay in the low GNRI group were significantly higher than those in the high GNRI group,and the difference was statistically significant(P<0.05).Con-clusion:Preoperative GNRI can predict the occurrence of postoperative complications in elderly SCI patients,clinical nurses need to timely carry out nutritional risk assessment on elderly SCI patients,and improve the nutritional status of patients before surgery to reduce the occurrence of postoperative complications.
