Clinical application of individualized continuous care for patients with chronic heart failure based on Donabedian quality theory system

Abstract：Objective:To observe the clinical application effect of Donabedian quality theory system in personalized extended care for patients with chronic heart failure(CHF).Methods:A total of 110 CHF patients admitted to the hos-pital from April 2021 to July 2022 were selected and divided into control group and observation group by random num-ber table method,with 55 cases in each group.The control group received routine care,and the observation group re-ceived individualized continuous care based on Donabedian quality theory system.The quality of life[minnesota quality of life scale for cardiac dysfunction(MLHFQ)],6 min walking test,left ventricular ejection fraction(LVEF)and read-mission rate were compared between the two groups.Results:After nursing,the MLHFQ score and readmission rate of the observation group was lower than that of the control group,and the 6 min walking distance longer than control group,LVEF were higher than that of the control group,the difference was statistically significant(P＜0.05).Conclu-sion:Individualized continuous care based on Donabedian quality theory system can improve the quality of life,improve cardiac function and reduce the rate of re-hospitalization in CHF patients.
