Biomarkers for early diagnosis of neuropathic pain in diabetes mellitus and nursing significance

Abstract:Objective:To search for biomarkers for early diagnosis of diabetic neuropathic pain(DPNP)in order to facilitate early diagnosis and individualized nursing intervention in DPNP patients.Methods:Patients with type 2 diabe-tes mellitus(T2DM)admitted to hospital from January 2022 to May 2023 were retrospectively analyzed and divided in-to DPNP group(53 cases)and DPNP group(76 cases)according to DPNP or no DPNP.The differences of blood glu-cose related laboratory indexes,cytokines and other indexes between the two groups were compared,as well as the cor-relation of each laboratory index.ROC curve was used to compare the value of different markers in DPNP diagnosis.Re-sults:The nerve growth factor(NGF)level in DPNP group was significantly lower than that in no DPNP group(P<0.05).Multifactor analysis showed that serum NGF level and tumor necrosis factor-α(TNF-α)level were independent risk factors for early diagnosis of DPNP.Correlation analysis showed that serum levels of NGF and TNF-α were closely correlated with glycosylated hemoglobin(HbAlc),fasting C-peptide(FCP),2-h postmeal C-peptide(2 h CP)and 24-hour urinary microalbumin excretion(UME)all P<0.05).ROC analysis showed that the area under the curve(AUC)of NGF was significantly higher than that of TNF-α(P<0.05).Conclusion:NGF and TNF-α are the key cytokines for early diagnosis of DPNP.In addition to active control of blood sugar,early individualized nursing interven-tions such as foot care,exercise care,health education and psychological care should be given to patients diagnosed with DPNP.
