Application of pharmaceutical care model of drug reorganization in diabetic patients

Abstract：Objective:To explore the effect of drug reforming pharmaceutical services on the rationality,compliance and multiple medication of diabetic patients during hospitalization.Methods:Through drug reform services for patients with diabetes in the endocrinology department of Liupanshui People's Hospital,the medical advice of 82 patients with diabetes who were currently applying and all drugs within 24 hours after admission or transfer were collected and sorted out.Results:All 64 of the 82 patients(accounting for 78％)had unreasonable drug use,and these patients were treated with therapeutic drug intervention.A total of 514 drug doctor orders were collected,of which a total of 198 drug doctor orders were wrong,with an intervention rate of 38.5％.Conclusion:Drug reform services have improved the drug com-pliance of diabetic patients during hospitalization,which improves patients'satisfaction with medical treatment;reduces the occurrence of adverse drug reactions and ensures the safety of patients'medication,greatly avoids repeated drug use,reduces the cost of drugs for patients,and reduces the economic burden of patients.
