Difference of clinical pharmacist guidance warfarin anticoagulation therapy and traditional treat-ment on non-valvular atrial fibrillation

Abstract:Objective:To observe difference on non-valvular atrial fibrillation between clinical pharmacist guidance war-farin anticoagulation therapy and traditional treatment. Methods:From December 2010 to December 2014,there are 58 cases of non-valvular atrial fibrillation,randomly divided into clinical pharmaceutical guidance warfarin treatment of 28 cases( guid-ance group)and non-clinical pharmaceutical guidance treatment group of 30 cases( traditional group). The guidance group&nbsp;was guided by the clinical pharmacists in the pharmacy service mode to guide patients with warfarin anticoagulant therapy,INR autonomous monitoring and dose adjustment,the traditional mode of the traditional group by the physician led warfarin antico-agulant therapy. Two groups target of international normalized ratio(INR)all are ranged from 2 to 3(75 years of age or older was 1. 5 ~ 2. 5),to monitor once INR for each interval 5 d at the hospital after the start of treatment. Two groups of patients started warfarin anticoagulation therapy for twelfth months to do a follow-up survey. Results:There was a significant difference between the two groups in the dosage of warfarin(P<0. 05). International standard value(INR)guidance group was(2. 54 ±0.05),the traditional group was(2.23±0.11).There were significant difference sbetweent he two groups(P<0.01).Self monitoring and dose adjustment,medication compliance and safety aspects of the follow-up survey,the results were signifi-cantly different(P<0. 05). Conclusion:Clinical pharmacists by pharmaceutical service mode to guide warfarin anticoagulation non-valvular atrial fibrillation patients are superior to traditional methods.
