Investigation and thinking of the current situation of polypharmacy in the elderly patients with chro-nic diseases

Abstract：Objective:To understand the situation of medication and current situation of polypharmacy of the elderly pa-tients with chronic diseases. Methods:To investigate the elderly hypertensive patients with diabetes mellitus in Zhenrucommuni-ty general outpatient servicefrom July 2015 to September 2015. Questionnaire contents include patients' basic information,sick-ness status and current medications. Results:All 6 002 valid questionnaires were received. Among them,5 877 people relied on medication treatment for a long time,accounted for 97. 92% ,60. 16% of the elderly patients took 5 or more than 5 kinds of medicines and health products. Conclusion:The polypharmacy phenomenon in elderly patients with chronic diseases in commu-nity is widespread. It is necessary to carry out the medication reconciliation in the community.
