Application of the hospital-community-family tripartite collaborative nursing model in elderly patients with severe diabetic foot

Abstract:Objective:To evaluate the application effect of the hospital-community-family tripartite collaborative nursing model in elderly patients with severe diabetic foot.Methods:A total of 84 elderly patients with severe diabetic foot admitting to our hospital from August 2023 to July 2024 were selected as the study subjects.Patients admitting from August 2023 to January 2024 were assigned to the conventional group,while those admitting from February to July 2024 were assigned to the study group,with 42 cases in each group.The conventional group received routine nursing care.The study group received nursing interventions based on the hospital-community-family tripartite collaborative nursing model in addition to the routine care.Blood glucose levels,affected foot condition,and nursing satisfaction were compared between the two groups.Results:After the intervention,the study group showed lower levels of fasting blood glucose,2-hour postprandial blood glucose,and glycated hemoglobin compared to the conventional group.The foot ulcer area and Visual Analog Scale pain score were smaller in the study group.The ankle-brachial index and nursing satisfaction were higher.There were statistically significant differences in terms of all the indicators(P<0.05).Conclusions:The implementation of the hospital-community-family tripartite collaborative nursing model in elderly patients with severe diabetic foot was beneficial in controlling blood glucose levels,improving foot ulcer healing,and enhancing patients'nursing satisfaction.
