Application of chlorhexidine whole-body bathing for patients with cardiogenic shock treated with extracorporeal membrane oxygenation combined with intra-aortic balloon pump

Abstract:Objective:To explore the effect of chlorhexidine whole-body bathing for patients with cardiogenic shock treated with extracorporeal membrane oxygenation(ECMO)combined with intra-aortic balloon pump(IABP).Methods:A total of 251 patients with cardiogenic shock admitted to the cardiac care unit(CCU)of a tertiary grade A hospital in Henan Province spanned form January 2021 to June 2024 were selected as the study subjects by convenience sampling.The 126 patients admitted spanned from January 2021 to December 2022 were assigned to the conventional group.Among them,57 patients who received only ECMO treatment were designated as separate intervention subgroup 1.The 69 patients who received combined ECMO and IABP treatment were designated as combined intervention subgroup 1.The 125 patients admitted spanned from January 2023 to June 2024 were assigned to the chlorhexidine group.Among them,54 patients who received only ECMO treatment were designated as separate intervention subgroup 2.The 71 patients who received combined ECMO and IABP treatment were designated as combined intervention subgroup 2.The conventional group received whole-body bathing with warm water,while the chlorhexidine group received whole-body bathing with 2%chlorhexidine gluconate.The incidence of bloodstream infection(BSI)during ECMO treatment,occurrence of complications,infection-related mortality,and pathogenic microorganisms in the patients'blood were analyzed.Results:The BSI incidence in separate intervention subgroup 2 was 11.11%,and the infection-related mortality was 5.56%.Both of BSI incidence and infection-related mortality in separate intervention subgroup 2 were lower than those in separate intervention subgroup 1(33.33%and 19.30%,respectively).The BSI incidence in combined intervention subgroup 2 was 23.94%,and the infection-related mortality was 15.49%.Both BSI incidence and infection-related mortality in combined intervention subgroup 2 were lower than those in combined intervention subgroup 1(50.72%and 31.88%,respectively).There were statistically significant differences in terms of the above indicators(P<0.05).Logistic regression analysis showed that whole-body bathing with 2%chlorhexidine gluconate was a protective factor against BSI occurrence.Conclusions:Whole-body bathing with 2%chlorhexidine gluconate could reduce the incidence of BSI and infection-related mortality in patients with cardiogenic shock.
