Application of multidisciplinary collaborative H2H nutrition management model in patients with liver failure

Abstract:Objective:To explore the effectiveness of multidisciplinary collaborative H2H nutrition management model in patients with liver failure.Methods:A total of 91 patients with liver failure hospitalized in an infectious diseases department of an infectious disease hospital in Wuxi city were selected as subjects by convenience sampling method.Among them,forty-five patients admitted from January to July 2023 were assigned to the control group.Forty-six patients admitted from August 2023 to February 2024 were assigned to the study group.The control group received routine nutrition management.The study group received the multidisciplinary collaborative H2H nutrition management model based on the control group.The nutritional status,liver function indicators,Model for End-Stage Liver Disease(MELD)score,incidence of gastrointestinal complications,unplanned readmission rate,and mortality were compared between the two groups.Results:During the study,3 patients in the control group and 4 patients in the study group dropped out.Finally,42 patients in each group completed the study.After the intervention,the Subjective Global Assessment(SGA)grade in the study group was better than that in the control group(P<0.05).The Royal Free Hospital-Nutritional Prioritizing Tool(RFH-NPT)score,albumin(ALB),prealbumin(PA),hemoglobin(HB),body mass index(BMI),mid-arm muscle circumference(AMC),and handgrip strength(HGS)were superior in the study group than those in the control group(P<0.05).In contrast,alanine aminotransferase(ALT),aspartate aminotransferase(AST),total bilirubin(TBiL),prothrombin time(PT),MELD score,total incidence of gastrointestinal complications,and unplanned readmission rate were lower in the study group than those in the control group(P<0.05).Conclusions:The multidisciplinary collaborative H2H nutrition management model could improve the nutritional status and liver function of patients with liver failure.It also could reduce the incidence of gastrointestinal complications and unplanned readmission rates of patients.
