Abstract:Objective:To systematically evaluate the impact of preoperative frailty on health outcomes in patients with esophageal cancer. Methods:Literature on the influence of preoperative frailty on health outcomes in patients with esophageal cancer were retrieved from China Biology Medicine Database,CNKI,Wanfang Database,VIP Database,PubMed,Web of Science,Cochrane Library,EMbase, and CINAHL.The retrieval time spanned from database establishment to March 24,2024.Literature was independently screened.Data were extracted, and study quality was assessed by 2 researchers based on inclusion and exclusion criteria.Statistical analysis was performed using RevMan 5.3 and Stata 17.0 software. Results:A total of 12 cohort studies were included.Meta-analysis results showed that preoperative frailty increased the risk of mortality(HR=1.79,95%CI 1.51-2.13,P<0.000 01),complications(HR=1.32,95%CI 1.16-1.49,P<0.000 1),and readmission(HR=1.24,95%CI 1.18-1.31,P<0.000 01)in esophageal cancer patients. Conclusions:Current evidence suggested that preoperative frailty in patients with esophageal cancer was associated with increased risk of adverse health outcomes.Greater emphasis should be placed on preoperative frailty assessment. The targeted interventions should be develop. The aim was to reduce the occurrence and progression of frailty, and to thereby lower the risk of postoperative adverse health outcomes.
