Application of disease diagnosis-related groups in nursing human resources and performance management

Abstract:Objective:To explore the feasibility of disease diagnosis-related groups in nursing performance management by establishing a mathematical model.Methods:The first page informations of medical records of 10 000 patients from 32 departments in our hospital from February 2022 to December 2023 were selected and summarized.Based on the grouping scheme released by the National Quality Control Center for Diagnosis-related Groups,the rank sum ratio(RSR)method was adopted to construct the evaluation model of diagnosis-related groups.Results:The top 8 departments in terms of RSR before being included in the disease diagnosis-related groups showed differences in treatment difficulty(CMI)and nursing intensity.Taking the intensive care unit(ICU)and the cardiac care unit(CCU)as examples,they performed relatively well in terms of CMI and nursing intensity.The emergency department,on the other hand,was relatively outstanding in terms of service breadth(the number of diagnosis-related groups).After the disease diagnosis-related groups,both the CCU and the department of cardiology ranked in the top two in terms of RSR.Compared with before being included in the disease diagnosis-related groups,their RSR all increased.Conclusions:Disease diagnosis-related groups could visually present the differences in treatment difficulty,nursing intensity and service breadth among departments.When used in nursing performance management,disease diagnosis-related groups could provide strong basis for the reasonable assessment of the value of nursing work.
