Application of high flow nasal cannula oxygen therapy in children's bronchoscopy examination

Abstract:Objective:To compare the oxygen therapy support effect between high flow nasal cannula oxygen therapy and double nasal cannula oxygen inhalation in children's bronchoscopy.Methods:Convenience sampling method was used to select 60 children who underwent electronic bronchoscopy in The Affiliated Xuzhou Children's Hospital of Xuzhou Medical University from October 2023 to March 2024 as the research subjects.They were randomly divided into high flow oxygen therapy group and nasal cannula oxygen therapy group by random number table method,with 30 cases in each group.The high flow oxygen therapy group was given nasal high flow oxygen therapy with an oxygen flow rate of 15-40 L/min,a temperature of 34℃,and the initial inhaled oxygen concentration(FiO2)of 40%.The nasal tube oxygen inhalation group was given oxygen inhalation with double nasal tubes,and the oxygen flow was 4 L/min.The incidence of intraoperative hypoxemia,the lowest percutaneous oxygen saturation(SpO2),peak intraoperative heart rate and respiratory rate,and incidence of intraoperative adverse events between two groups were compared,then to observe and record the number of cases of FiO2 elevation and the average FiO2 in the high flow oxygen therapy group.The number of cases of FiO2 increase and the average FiO2 in the high flow oxygen therapy group were observed and recorded.Results:The incidence of intraoperative hypoxemia and peak respiratory rate in the high flow oxygen therapy group via nasal cannula were lower than those in the nasal tube oxygen inhalation group.The lowest intraoperative SpO2 was higher than that in the nasal tube oxygen inhalation group,and the differences were statistically significant(P<0.05);There was no statistically significant difference in the peak intraoperative heart rate and incidence of adverse events between the two groups(P>0.05);The number of cases with elevated FiO2 in the high flow oxygen therapy group was 16,and the average FiO2 was(50.67±15.52)%.Conclusions:Compared with nasal cannula oxygen therapy,high-flow nasal cannula oxygen therapy for children undergoing bronchoscopy could reduce the incidence of intraoperative hypoxemia and maintain higher oxygen saturation and more stable respiratory status in children.
