Influence of frailty on medication differences in the hospital-home transition in elderly patients with cardiovascular disease based on propensity matching score

Abstract:Objective:To investigate the impact of frailty on medication differences(MD)during the hospital-to-home transition in elderly patients with cardiovascular disease(CVD).Methods:A convenience sample of 231 elderly CVD patients was recruited in a tertiary grade A hospital from February 2022 to February 2023.Prior to discharge,socio-demographic characteristics of the subjects were collected using a general information questionnaire,while frailty assessment scale and Medication Deviation Instrument were used to evaluate the frailty and MD of the subjects on the tenth day after discharge respectively.Propensity score matching(PSM)was utilized for analyzing the impact of frailty on MD among elderly CVD patients during their transition period from hospitalization to family care.Results:The incidence of frailty among elderly CVD patients was 32.90%,and the incidence of MD during the hospital-to-home transition period was 75.76%.After PSM,frail elderly CVD patients had a 4.978-fold higher risk of MD compared to non-frail patients(OR=4.978,95%CI 1.616-15.340).Conclusion:The prevalence of frailty and MD during the hospital-to-home transition is higher among elderly CVD patients,with frailty having an impact on MD occurrence.Medical staff should enhance screening for frail elderly CVD patients and actively promote quality management of medication during the transitional period between hospitalization and home care.
