Application of multidisciplinary combined with five-step progressive activity programme in patients with mechanical ventilation in ICU

Abstract:Objective:To explore the application effect of multidisciplinary combined with five-step progressive activity programme in patients with mechanical ventilation in intensive care unit(ICU).Methods:Patients with mechanical ventilation in ICU admitted to a hospital in Qingdao were selected as study subjects from May 2022 to July 2023.According to the admission time,the patients were divided into control group with 49 cases and intervention group with 48 cases.The control group was intervened with routine treatment and nursing methods for severe cases,while the intervention group was intervened with multidisciplinary combined with five-step progressive activity programme.The incidence of ICU-acquired weakness(ICU-AW),activities of daily living,incidence of ICU-acquired delirium,duration of mechanical ventilation,length of ICU stay,and incidence of lower limb deep vein thrombosis between two groups of patients were compared.Results:The Barthel index of patients in the intervention group was(29.48±14.26)points,which was higher than that in the control group.The Barthel index of patients in the control group was(17.14±14.25)points.The duration of mechanical ventilation in the intervention group was(5.56±3.75)days,and the length of ICU stay in the intervention group was(8.15±3.94)days,which were shorter than those in the control group.The duration of mechanical ventilation of control group was(7.92±6.28)days,and the length of ICU stay in the control group was(11.69±8.73)days.The incidence of ICU-AW in the intervention group was 17%,the ICU-acquired delirium in the intervention group was 2%,and the incidence of lower limb deep vein thrombosis in the intervention group was 21%,which were lower than those in the control group.The incidence of ICU-AW in the control group was 37%,the ICU-acquired delirium in the control group was 27%,and the incidence of lower limb deep vein thrombosis in the control group was 49%.The differences were statistically significant(all P<0.05).Conclusions:Implementing multidisciplinary combined with five-step progressive activity programme is beneficial for improving activities of daily living of patients with mechanical ventilation in ICU,reducing the incidence of ICU-AW,ICU-acquired delirium,incidence of lower limb deep vein thrombosis,shortening duration of mechanical ventilation and length of ICU stay.
