Evidence-based practice of intermittent pneumatic compression to prevent deep venous thrombosis of lower extremities during perioperative period in patients

Abstract:Objective:To apply the best evidence of intermittent pneumatic compression(IPC)in preventing deep vein thrombosis(DVT)of surgical patients and to evaluate its effectiveness.Methods:The best evidence of safe use IPC during perioperative was summarized according to the standard procedures of Joanna Briggs Institute clinical evidence practice application system,and corresponding clinical review indicators based on the best evidence were developed.The obstacles were analyzed,and action strategies were formulated.The cognitive level of IPC protection among operating room nurses,the diameter and blood flow velocity of the popliteal veins in both lower limbs of patients before and after surgery,the level of D-dimer in patients,the incidence of DVT in patients 24 hours after surgery,and the implementation rate of review indicators of nurses before and after the application of the best evidence were compared.Results:The score of operating room nurses' knowledge of IPC protection in round 2 quality review was(83.58±8.78),which was higher than the score(44.58±12.93)in the first round of baseline quality review.Postoperative blood flow velocity in the popliteal veins of both lower extremities was faster in round 2 quality review than in the first round of baseline quality review,and D-dimer was 1.25(0.86,3.15)mg/L at 24 h postoperatively in round 2 quality review,which was lower than that[2.25(0.91,5.13)]mg/L in the first round of baseline quality review.The incidence of DVT at 24 h postoperatively in round 2 quality review patients was 2.50%,which was lower than that(11.54%)in the first round of baseline quality review.The rate of implementation of review metrics 1 to 20 by nurses in round 2 quality review was higher than that in the first round of baseline quality review,and all differences were statistically significant(all P<0.05).Conclusions:Based on the best evidence,implementing perioperative IPC for patients is beneficial for standardizing nursing behavior and reducing the incidence of lower limb DVT in patients.
