Research and application progress case management on stroke patients
Abstract：This article reviewed case management on stroke patients from application model,core personnel,implementation process,effect evaluation and key measure.It pointed out that through learning from foreign experience and standard,the case management system on stroke patients which suited for situation in China could be built.
