A systematic review of transitional care model to reduce the re-admission rate of elderly patients with chronic diseases in China
Abstract：Objective:To carry out the systematic review of the application of transitional care model to reduce readmission rate of the elderly patients with chronic diseases in patients.Methods:The randomized controlled trials of the transitional care model in elderly patients with chronic diseases in China were retrieved in Wanfang Database,China Knowledge Network,Web of Science Database.The retrieval time was from the establishment of database to July 1,2016,in strict accordance with the inclusion criteria to screen literature,extract data and evaluate quality.Results:A total of 9 articles,including Chinese 7,English 2;5 middle quality literature,4 low quality literature,involving 2 442 patients;patients follow-up was conducted either by telephone follow-up,family visit,follow-up visit or a combination of two of them;intervention antinued 3 months to 6 months,the hospitalization rate was significantly reduced,and the intervention effect was stable.Conclusions:Intervention for 3 months to 6 months of transitional care model could reduce the re-admission rate of elderly patients with chronic diseases in China,but a system of transitional care model had not been yet formed in China.
