Comparison research on prognosis ability of hospitalized patients in Emergency Department by application of im-proved early warning score and APACHE Ⅱ evaluation
Abstract:Objective:To compare the feasibility and applicability of predicting the prognosis of hospitalized pa-tients between Modified Early Warning Score(MEWS)and Acute Physiology and Chronic Health Evaluation Ⅱ(APACHE Ⅱ)two scoring tools for in department of emergency. Methods:A total of 640 inpatients in depart-ment of emergency were scored with APACHE Ⅱ and MEWS by using prospective study, and to follow the prognosis of all patients and compare the corresponding indicators including sensitivity, specificity, positive pre-dictive value, negative predictive value and ROC curve for predicting the prognosis resolution. Results:In aspect of predicting patients’mortality risk,MEWS scoring had higher resolution. The area under ROC curve in MEWS score was 0.93.The area under ROC curve in APACHE Ⅱ score was 0.79,and there was statistically signifi-cant difference(Z=4.348,P<0.01).Conclusion:Both MEWS score and APACHE Ⅱ score could be used to j udge the severity of emergency patients, and it had a certain predictive value for the patients’mortality risk. But the MEWS score was simple and quick to operate, and could be used as a useful supplement for APACHEⅡ score.
